Before Your Appointment
Print and complete the Patient Information and Medical Questionnaire forms located on the Resources and Forms page and bring them to the office. You may also complete the form at our office. Completing the forms before your appointment will allow faster processing and maximization of your appointment time.
If you would like to use our Patient Portal service, complete the Portal Authorization form as well (located on the same page). For more information on our portal service, visit the Portal page.
Bring printed copies of any forms that you have filled out with you to the office. We strongly recommend completing all documentation before visiting the office.
Make sure you bring a valid, government-issued ID for verification.
We accept most insurance plans. Check with your insurance if we are in your insurance network. It is crucial that you bring your insurance card(s), and Medicare/Medicaid card(s) with you to your appointment to help us with timely and accurate filing of your claims.
Many insurance plans require a written referral from your Primary Care Practitioner. Please call your Primary Care Practitioner's office to confirm that the referral is completed and in our office at the time of your appointment. Our fax number is (425) 899-2220. If we do not have your referral, we may need to reschedule your appointment.
We request office visit co-payment at the time of your visit. We accept cash and checks. Credit and Debit Cards are not accepted.
Please bring a list of all medications, prescription and over-the-counter, you are currently taking. Please contact your Primary Care Practitioner or referring physician to fax any recent clinic notes and laboratory/radiology tests to our office to assist us in delivering appropriate care.

To cancel your appointment, please call our office at least 2 business days in advance of your scheduled appointment. A missed appointment without such notice will incur a fee of at least $50. Some types of appointments can be cancelled through the patient portal. We request that you call the office and let us know about your cancellation prior to cancelling your appointment online.
Cell Phone Use
During you visit, we ask that you refrain from cell phone use and put your rigner on silent or vibrate as a courtesy to others and to our staff. We appreciate your cooperation in this matter.
Appointments on the Patient Portal
You may view scheduled appointments on the patient portal. Any appointment that appears, whether confirmed or unconfirmed, is a scheduled appointment. Missed appointments that appear as "unconfirmed" on the patient portal will incur the missed appointment fee (at least $50). Some types of appointments can be cancelled online. We request that you call the office and let us know about your cancellation prior to cancelling your appointment online. At this time, online appointment scheduling services are not available. Please contact the office to schedule an appointment.
